Problem Submitting 4k Footage

I am curious if anyone has a solution for my issue. I upload my 4k footage and fill in all fields and it returns a the following error: "There were errors in Source" what does this mean and what is the solution?

I believe the 'source' is a field where you say whether it is from a camera or graphics..check all the fields that you need to fill in and you should find it
