Disappearing Keywords and Categories

Anyone else having trouble with disappearing keywords as well as categories? This is happening randomly with my most recent footage as well as footage that's been listed for months.

All is well and then suddenly some clips don't have keywords or a category is not chosen. Pretty obvious when you view your files on the upload page and scroll through your list and see the big empty spaces that were once filled. Click to the clips page and sure enough, no keywords where keywords once were.

Anyone else seeing the same? Could it be that they are there but just not showing up?

It has happened in the past and I also had times a year ago or so when extra keywords were added.
