Need assistance with licensing agreement

Hello everyone,

I need your assistance with a licencing agreement for footage.

I was contacted by a studio about some of my footage and this is what they want:

"We were interested in licensing some of your footage. Do you have any agreement for licensing fotoage? We would need all rights, in all media, worldwide in perpetuity. I can be reached via e-mail or phone and look forward to hearing from you soon.

Two questions:

1) Is there a standard document for this type of transaction ?

and

2) How do you price footage in this instance?
