Error when attempting to download a free clip

I have downloaded clips from Pond5 before, but none on my own account. I attempted to download this week's free clip (an ostrich walking in Africa), but I keep getting an error message that says my address is incomplete. I filled out all the fields that have red asterisks, but keep getting an error message that says I need to complete the form.

Is there any obvious catch with this form? Again, every field with a red asterisk is filled out with correct information.
