Universal Model Release?

I was wondering if there was a model release that can be used through all the different agencies or does each agency require its own specific format?

The release form provided by Pond5 works for me at P5 (of course), CC, CS, RS, and I "think" at SS. (the last I say "I think" because it's been a while...)

I use a generic one with my company name on top. All sites accept it.

Is yours less scary to the wary? It is not uncommon for the general public to shy away from the document, and I REALLY suck at explaining it. (I'm not sure which is worse - me, or the terms of the release)
