Model release form date

I intend to upload model release forms for my husband, baby and myself. I have to go to a friends house to use their scanner though and as I don't want to be doing this EVERY time I have a video, can I put the photo shoot dates as "May 23, 2011- May 24, 2011" or something? So that I can upload the same form with all my videos for awhile?

Since the release is digitized anyway, save as a jpg, open it in Photoshop, or any image editor with text editing ability, and use that to insert the appropriate date. I've even gone so far as to isolate oft-used signatures since I don't have easy access to a scanner or a FAX.

Are we required to upload a release form per model per session?
or just once per model the first time for initial filing?
